Grace Hill provides comprehensive primary care services as well as support services to all the public housing residents in zip codes 63107, 63106 and 63104 areas through the Grace Hill Murphy-O’Fallon Health Center located on Biddle Street, Hadley Health Center located on Hadley and the Soulard-Benton Health Center located on Lemp Avenue. 

 

Grace Hill also has extended its primary care services and outreach to the residents of Section 8 housing.  Currently, Grace Hill serves more than 6,500 residents of public housing and Section-8 developments in the City of St. Louis.

 

Grace Hill has also developed an “Integrated Health and Support Services Model” which includes primary and preventive health care, dental services as well as health support services which include an on-site pharmacy, transportation, medical social services, x-ray and mammography services, WIC and on-site eligibility verification and enrollment by DFS workers. Grace Hill’s community health services include In-Home Services for elderly, pre-natal case management, High Risk Infant follow up, Health Education, Promotion and Wellness.

 

Grace Hill partners with The St. Louis Housing Authority to provide affordable primary care services to its residents.

 

Grace Hill partners with The Kingdom House to provide peer counseling services to the teens in the Near South Side.
